A concept map records how concepts in one system relate to concepts in another: this local code corresponds to that SNOMED CT concept; this legacy procedure code is broader than that current one. In FHIR (Fast Healthcare Interoperability Resources) the ConceptMap resource holds such mappings as groups of elements, each stating a source code, a target code, and a relationship from a fixed list (related to, equivalent, source is narrower than target, source is broader than target, not related to), with optional comments and dependencies on other codes.
The scope note lists what makes a mapping usable rather than merely present. A mapping has a direction: that A maps to B does not by itself say that B maps to A. It has a relationship type, and "equivalent" is a strong claim that most mappings cannot honestly make. It applies between stated versions of the source and target systems, and may become wrong when either is updated. And it has an author and a basis, which is why NucLex's reserved mapping schema includes evidence, author, confidence, and review state.
In this publication's language the word concept map is used in this terminology sense only, not in the educational sense of a hand-drawn diagram of ideas. NucLex entries carry no mappings in W0; the entry on Identifier explains why. The FHIR R5 ConceptMap page is the authoritative reference.
